Although a small number of players may have 5+ years of beginner adult experience, the majority of teams consist of players with 1-5 years entry level playing experience. Comparable to "E" level adult league teamsIntermediate - This division may have players with some organized playing experience (either as an adult or youth), little to no youth playing experience, but have played several years as an adult player, and players that have some non-varsity high school experience but are several years removed from those levels. Comparable to "D" level adult league teams.Bronze - This division is comprised of players that may have experience at the Division II or III ACHA levels, Junior B or C, Varsity High School, and lower midget levels. Some players have limited organized youth playing experience, but have several years of experience as a recreational adult player. Comparable to "C" level adult league teams.Silver - This division consists of advanced level players who may have Junior A or B, Non-Varsity College (ACHA), Midget Major AAA, and Prep/Varsity High School playing experience. Player Info and ConsentName First Last Military Branch*Military BranchEmail PhoneWarriors Code of Conduct*Due to the size, visibility and formal event participation of Avalanche Warriors Ice Hockey (AWH), the Board of Directors has decided to require a "Code of Conduct" Policy for all Players, Coaches, Team Personnel, Volunteers and Team Supporters consistent with the AWH Mission Statement. SPORTSMANSHIP: All Players, Coaches, Support Staff, Volunteers, and Team Supporters are expected to conduct themselves in a sportsmanlike manner. We are all here to participate in recreation and show good sportsmanship. While we encourage people to support their Team and/or Teammates with enthusiasm, use of vulgar, rude or inappropriate comments or gestures will not be tolerated. ALCOHOL AND DRUG USE: Aside from prescribed drug use, illegal and/or unauthorized drug use at an AWH sponsored game or practice shall not occur at the rink facilities. Alcohol shall not be consumed during the game. Alcohol may be consumed before or after; pending rink and/or tournament policy. Unauthorized drug and/or alcohol use is subject to discipline by the AWH Discipline Committee and Board of Directors of AWH. CONDUCT OUTSIDE OF LEAGUE ACTIVITIES: Sportsmanship extends beyond "Game Day" activities. Inappropriate comments whether verbal or written including on social media can reflect negatively on AWH and its Players and may be subject to disciplinary action by the AWH Discipline Committee and the Board of Directors of AWH. VIOLENCE: Violence will not be tolerated. There is a policy governing fighting in the rules of USA Hockey. Coaches, Support Staff, Volunteers and Fans who fight or commit an act of violence or in the opinion of the Referees are unruly, will be asked to leave the premises and may be banned from attending future AWH games or events. REFEREES: The Referees are in "Charge" of calling the game. Only a Coach, through a Team Captain, or the Team Captain, if no Coach is present, may question a call on the ice. SUPPORTERS OF THE TEAM: If a Referee feels a Team Supporter or Supporters is being vulgar, inappropriate or unruly, it is within the right of the Referee to ask the Coach or Team Captain, if no Coach is present, of the team the Supporter or Supporters is rooting for, to speak with the team Supporter or Supporters. If necessary, the Supporter or Supporters may be asked to leave the arena and could be banned from attending future games of the team. DISCIPLINARY ACTION: The Board of AWH and the AWH Discipline Committee reserve the right to suspend, censure or otherwise discipline any Player, Coach, Volunteer or Team Personnel who is deemed to have violated any part of the Code of Conduct. A Player subject to discipline may petition the Board of the AWH for reversal or modification of the disciplinary action. I have chosen to play for the Avalanche Warriors for the 2019-2020 USA Hockey covered seasons. I have read this Code of Conduct. I understand the commitments and sacrifices that will be asked of me as a member of my team. I promise to abide by the Code of Conduct and will do everything I can to uphold the mission of AWH while playing and representing the team and will do everything can to help ensure the success of the team and the program through my active participation in practice and playing schedules. I understand there is a risk of injury from participating in the sport of hockey. I knowingly and freely assume all such risks and will not hold the AWH Board, Coaches, Volunteers, Officials or Sponsors liable for any injury I might incur. I will keep my equipment in proper condition and wear the equipment according to the AWH requirements. I will maintain a current USA Hockey Registration number for the current season. I hereby give my consent to AWH and its Agents to photograph and film myself while playing in AWH sponsored games or practices for media use, and without limitation to use any pictures, films and/or stories in connection with AWH activities without consideration of any kind. I agree to the Warriors Code of ConductInjury/Concussion Consent Form*Hockey is a rough game in nature. Despite the fact that we are playing a non-check, adapted version of the game, injuries are bound to occur playing ice hockey. We also realize that because we are dealing with wounded, injured, and otherwise disabled veterans, this further increases the risk factor. If a player is injured on the ice, proper safety precautions will be taken as necessary. Ultimately, however, players participate at their own risk and Avalanche Warriors and/or any of its members are NOT responsible for any liability and you agree not to seek any compensation from the team and/or players. Players are required to wear (at a minimum) Helmets, athletic cups, shin pads, and gloves for practices. For games, players will wear (at a minimum) helmet, pants (breezers), elbow pads, athletic cup, shin pads, and gloves. Mouth guards and cages are not required but are highly encouraged. If a player is injured on the ice or away from the team, it is the responsibility of the player to keep their captain informed of the situation. Failure to do so may result in placement on the inactive roster. Depending on the seriousness of injury/concussion, team captains will have the final call.
